Students

Anyone with information about hazing activities should report that information using the UCLA Public Incident Report Form.

Report Hazing

If you are looking to report an instance of hazing that is currently underway and needs immediate response, please contact UCPD at (310) 825-1491.

When Reporting an Incident:

UCLA's ability to investigate reported incidents, enforce the university's expectations, and protect student safety depends on the accuracy and specificity of the information provided. You are encouraged to provide as much specific detail as possible so that appropriate action can be taken to address the reported behavior.

The form is not inherently anonymous as it does ask for name and email address. You may create a non-identifiable email address/name to ensure anonymity, however, please be aware that the University may be unable to complete an investigation and/or hold students/organizations accountable without the ability to share and evaluate the spruce to the property for credibility.

Retaliation against any persons who reports misconduct, assist someone with a report of misconduct, or participates in any manner in an administrative review or resolution of a misconduct report is prohibited under both the Student Code of Conduct and the Student Group Code of Conduct.

Faculty/Staff

The University of California's Abusive Conduct Policy prohibits abusive conduct and retaliation by and against members of the University community in the workplace. It applies to all University employees(including student employees), unpaid interns, and third parties. The Policy prohibits retaliation against any person, in good faith, reports abusive conduct, assists someone with a report of abusive conduct, assists someone with a report of abusive conduct, or participates in an investigation or other process under the Policy. Abusive conduct may be reported via the UCLA Abusive Conduct Intake Form.

Submission:

Grievances, complaints, or abusive conduct forms may be filed via email to labor.relations@chr.ucla.edu or delivered by mail or in person to:

What Happens After Reporting:

After a report of hazing is made, the routing is determined by who is alleged to have engaged in the prohibited hazing conduct.

When the alleged prohibited hazing conduct involves:

  • UCLA Students or Student Organization, the Office of Student Conduct reviews the report and determines the appropriate action.
  • UCLA Staff, Campus Human Resources reviews the report and determines the appropriate action.
  • UCLA Faculty, the Academic Affairs and Personnel Office reviews the report and determines appropriate action.
  • Non-Affiliates, the report will be shared with the UCLA Compliance Office for review.
